I wish I had access to this app. years ago. As primary caregiver for my mother, navigating her long road from occasional forgetfulness to eventual full blown dementia was extremely difficult. There is so much helpful information here. There are important guidelines that include how to make a safe home environment for a loved one, how to advocate for them with their Drs./medical staff, to navigating family discussions on the patient’s care, finances, medications. I learned much of this the hard way, from hands on experience. This information would have made things much easier for me, my mother, and our entire family.

Went through the different aspects of this app which is particularly useful for people coping with care of a person having Alzheimer’s symptoms.
This app not only channelizes and presents crucial information about Alzheimer’s care resources, but also provides guidance on a variety of related aspects.
In short, Alzheimer’s Care Partner is a boon for all those caregivers who need a handholding partner in their quest for better structured care for Alzheimer’s patients.

Dementia affects not only the individual with the disease, but all of that individual’s loved ones. It is important to have a resource such as Alzheimer’s Care Partner where one can log medical information, symptoms, and coordinate care with other family members and caregivers so that important decisions can me made and more informed engagement with physicians, nurses, social workers among others in the health care system can occur.

This app allows families to easily track dementia-related issues and coordinate care between family members and paid caregivers while providing evidence-based knowledge to best navigate the many decisions that need to be made. - Monthly Subscription: $9.99/month billed monthly - Track disease progression and share reports with doctors so you can evaluate treatments, plan ahead and improve the quality of life for the person living with dementia.
- Basic – Free: Only shows last month’s data, so health trends are limited. Reports can only be viewed in the app — no sharing option with doctors or caregivers.
For the paid plans you will be charged to your Apple Account, which will automatically renew unless canceled at least 24 hours before the end of the current period. Auto-renew can be turned off anytime by going to your Account Settings after purchase. No cancellation of the current subscription is allowed during the active subscription period.
