The CardioNexus App is a secure digital tool designed to help patients share health information directly with CardioNexus' nursing team. It serves as a communication bridge, allowing users to upload relevant data that can assist healthcare professionals in assessing heart health and supporting ongoing care.

Importantly, the CardioNexus App is not intended to diagnose, monitor, screen, prevent, treat, or make decisions about a patient’s heart condition. Its primary function is to facilitate the flow of information to qualified health professionals who are responsible for interpreting and acting on that data.

Any monitoring or follow-up is conducted by real people, experienced nurses and cardiologists who review the submitted information and, where appropriate, update your cardiologist to ensure continuity of care.

The CardioNexus App is designed for patients who want to be engaged in their cardiac care journey by securely sharing updates, symptoms, or test results with their care team, while maintaining confidence that all assessments and decisions are made by qualified clinicians.

Seamless Integration with Apple Health
To provide your care team with a more complete view of your daily well-being, the CardioNexus App integrates with Apple Health. With your permission, you can securely share relevant health and wellness metrics with your care team. This objective information helps both you and your care team monitor your daily well-being and is available for you to review at any time in the Progress tab. This data complements the updates you provide manually, giving your clinical team a richer, more holistic understanding of your health journey.
