App Profile: CareMappr Medical Data Maps

Note: This app is only available for individuals signed up by their clinician or carers signed up by a consumer.

CareMappr is designed to help assist in care management by providing you with your health data in one place.

The app asks simple questions during the week to help you understand your health journey, which can be shared with your health professionals to better assist you.

Carers can be invited to give an outside view of your wellbeing to give you insights you would not otherwise see.
