App Profile: Intercept

Intercept is designed to help patients who had a recent cardiac event, self-manage their health. With access to a range of features, the app supports rehabilitation and provides tools to track wellbeing, build daily schedules, and add goals. Intercept facilitates self-management and monitoring outcomes: healthier lifestyles, improved risk factor control and adherence with cardioprotective medications.

Patients can also record their daily step count from Apple Health if they want to.
