PSI App allows students to get cost estimates on their preferred insurance plan without registration. Current PSI customers can securely access their PSI account to view their plan summary and get information about what's covered. More importantly, PSI customers can download the following insurance documents:

5) Insurance Waiver Form or Waiver Instructions
