Calendar option for selecting date of birth I dysfunctional: One can select the day of birth, but NOT the month or the year ! ! ! I had to calculate the number of months that had a lapsed between that of my birth and the month I was filling out the form, and then swipe them backwards To get to the month in which I was born! Otherwise the form on the app worked perfectly.

I am now at least 3 hours in with being on the phone with UA. I have emailed StPH at least five times. A trip that I’ve been planning for months might be dramatically changed due to the terrible design of the data entry platform used by StPH. I put in the correct date of arrival and the program would change it without me knowing. It even changed my DOB but luckily I was able to catch that. If you go back the program wipes out everything you had previously entered and because there are numerous pages you can’t view your completed form in its entirety before you click to complete. I am locked out with no way to re-enter an arrival date. This has been a nightmare. I am at risk of not being able to board my flight. I love Spain and would think there would be platform designers out there that would be on top of this???? Sorry guys/gals. I never write bad reviews but this warrants a bad review.

I started filling out the form (as a user with no EU digital COVID cert) and noticed the following issues. I started filling it out too early, but no prob I just waited until 72 hours out. I got a notification at that time to continue the form, but when I finished it an error message informed me that it was still not within the 72 hour window (technically not correct!). At 48 hours out I got the same notification and continued. You must enter the flight arrival date on the first screen but when it is repeated back to you on the second screen it has changed to one day earlier. I’m not sure if I should leave it or enter the wrong date so it changes to the correct date. Please fix bugs asap!!!!!!

The Government of Spain has established a series of health control measures, including the completion of a health form prior to travelling to Spain, and the presentation of the QR code associated with said form when entering the country at any airport. This application allows you to:
- Fill in the form
- Obtain the QR 72 hours before arrival in Spain
- Manage your QR and show it to airport control authorities when you enter Spain
The form must be completed by all persons flying to Spain from countries that do not belong to the European Union or are not considered Schengen countries, if they do not have an EU COVID Certificate or EU Equivalent. Each form is personal and non-transferable and is associated with a single trip.
